Evil germs are everywhere and out to get us \u2013 especially the innocent, well-dressed children playing nearby. The smart ones among us, the marketers tell us, navigate this ominous world armed with the right sanitizing defense. Even the grimiest restroom, the ad images show, can become as innocuous as a sparkling, surgery-ready space if we only have the security of hand sanitizer. Yes, the power of the imagination\u2026<\/p>\n<p><span id=\"more-10074\"><\/span><\/p>\n<p>Sure, the marketing gurus make their buck by coaxing rampant, misplaced fear in the populace. (Funny how we don\u2019t seem as terrified by a \u201cBiggie\u201d order of French fries as we do our own door knobs.) However, let\u2019s put aside the cautionary ads and look at the facts. Sales have <a title=\"Hand Sanitizer: Good Hygiene or Just Hype?\" href=\"http:\/\/abcnews.go.com\/GMA\/Weekend\/america-obsessed-hand-sanitizer\/story?id=9260788\" >risen some 70% since last year<\/a>. Part of it is the whole <a title=\"How Not to Get Swine Flu\" href=\"http:\/\/www.marksdailyapple.com\/how-not-to-get-swine-flu\/\" >H1N1 alarm<\/a>, but it\u2019s a general trend as well. Even if the <a title=\"Ed Hardy Designer Hand Sanitizer\" href=\"http:\/\/springwise.com\/fashion_beauty\/edhardyhabit\/\" >craze<\/a> isn\u2019t warranted, what about more moderate use of the products? Do these things even work? Do they really keep us healthier? Are we breeding so-called \u201csuperbugs\u201d every time we use a squirt or spray? As always, let\u2019s break it down.<\/p>\n<p>The vast majority of hand sanitizers out there are alcohol based (like Purell) with one or more of the typical n-propanol, isopropanol or ethanol. To be effective, they contain anywhere from 60-90% alcohol. The alcohol \u201ckills\u201d (i.e. deactivates) the germs but requires the full drying time to do so. Additionally, sanitizers offer some ongoing, residual protection by making the skin surface inhospitable for bacteria and viruses. Ethanol-based sanitizers do a better job at <a title=\"Improved Inactivation of Nonenveloped Enteric Viruses and Their Surrogates by a Novel Alcohol-Based Hand Sanitizer\" href=\"http:\/\/aem.asm.org\/cgi\/content\/abstract\/74\/16\/5047\" >killing certain viruses like the norovirus<\/a>. Natural sanitizers like CleanWell use extracts from herbs (thyme being one) with inherent antiseptic properties and show effectiveness rates similar to chemical-based products (<a title=\"CleanWell\" href=\"http:\/\/www.cleanwelltoday.com\/downloads\/CleanWell_Ingenium_Techbrief.pdf\" >PDF Press Release<\/a>).<\/p>\n<p>Next, <strong>are you less likely to get the cold or flu if you use them regularly?<\/strong> Research suggests that <a title=\"The Effect of Hand Hygiene on Illness Rate Among Students in University Residence Halls \" href=\"http:\/\/www.fda.gov\/ohrms\/dockets\/dailys\/03\/Sept03\/090203\/75n-0183h-c000075-att-14-vol154.pdf\" >college dorm residents<\/a> (PDF) and <a title=\"Alcohol-free instant hand sanitizer reduces elementary school illness absenteeism.\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11039151?ordinalpos=1&amp;itool=EntrezSystem2.PEntrez.Pubmed.Pubmed_ResultsPanel.Pubmed_SingleItemSupl.Pubmed_Discovery_RA&amp;linkpos=1&amp;log$=relatedarticles&amp;logdbfrom=pubmed\" >elementary students<\/a> given instruction and ready access to hand sanitizers show fewer overall infections and absentee rates than control groups. (It\u2019s worth noting that the elementary school students used an herbal, alcohol-free sanitizer.) One <a title=\"Comparing hand washing to hand sanitizers in reducing elementary school students' absenteeism.\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17907739?ordinalpos=1&amp;itool=EntrezSystem2.PEntrez.Pubmed.Pubmed_ResultsPanel.Pubmed_SingleItemSupl.Pubmed_Discovery_RA&amp;linkpos=1&amp;log$=relatedarticles&amp;logdbfrom=pubmed\" >randomized cross-over comparison<\/a> of <strong>traditional hand-washing measures versus sanitizer use in an elementary school showed similar effectiveness in reducing absenteeism<\/strong>.<\/p>\n<p><strong>What are the cons of these products?<\/strong> Although alcohol-based sanitizers appear to be easier on the skin than soap and water washing, <a title=\"Adverse reactions associated with an alcohol-based hand antiseptic among nurses in a neonatal intensive care unit.\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12548257?ordinalpos=1&amp;itool=EntrezSystem2.PEntrez.Pubmed.Pubmed_ResultsPanel.Pubmed_SingleItemSupl.Pubmed_Discovery_RA&amp;linkpos=3&amp;log$=relatedarticles&amp;logdbfrom=pubmed\" >some people react to the products<\/a>. Aside from the alcohol itself, other ingredients can be suspect, particularly the fragrances used in some formulas. There\u2019s also some concern about the use of ethanol-based sanitizers, particularly on children. A safety review out last year suggested <a title=\"Safety evaluation of topical applications of ethanol on the skin and inside the oral cavity.\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19014531?ordinalpos=1&amp;itool=EntrezSystem2.PEntrez.Pubmed.Pubmed_ResultsPanel.Pubmed_SingleItemSupl.Pubmed_Discovery_PMC&amp;linkpos=2&amp;log$=citedinpmcarticles&amp;logdbfrom=pubmed\" >additional research was necessary to confirm topical safety<\/a> for regular use products. Herbal-based sanitizers don\u2019t dry hands as much as alcohol-based products, and they\u2019re generally viewed as safe options across the board. One up and coming category I\u2019d definitely recommend skipping is the silver nanoparticle sanitizer. The research shows <a title=\"Nanomaterials and nanoparticles: Sources and toxicity\" href=\"http:\/\/74.125.95.132\/search?q=cache:Zzz1tp82dWcJ:arxiv.org\/pdf\/0801.3280+nanoparticle+silver+santizer&amp;cd=6&amp;hl=en&amp;ct=clnk&amp;gl=us&amp;client=firefox-a\" >too many risks<\/a> to make these products worthwhile by a long shot.<\/p>\n<p>As for the risk of encouraging \u201csuperbugs,\u201d the whole of the research suggests that sanitizers are in the clear. The real culprit here is <a title=\"Triclosan: What the Research Shows\" href=\"http:\/\/www.beyondpesticides.org\/antibacterial\/triclosan-research-3-09.pdf\" >triclosan<\/a> (PDF), an anti-bacterial ingredient found in everything from anti-bacterial soaps to mouthwash to lunch boxes to running shoes (look for the \u201cmicroban\u201d label). It\u2019s a potent endocrine-disruptor. If you have a choice of washing with a triclosan-based soap or using a regular hand sanitizer, I\u2019d go with the sanitizer \u2013 hands down. (Sorry \u2013 couldn\u2019t resist.)<\/p>\n<p><strong>Personally, I don\u2019t think sanitizers are necessary for normal, everyday circumstances<\/strong>, but I don\u2019t see <em>too<\/em> much harm in them, particularly the natural herb-based brands. Maintaining a <a title=\"A Sanitized World is a Healthier World?\" href=\"http:\/\/www.marksdailyapple.com\/immune-system-strength\/\" >strong immune system<\/a> and using <a title=\"Hand washing: Do's and don'ts\" href=\"http:\/\/www.mayoclinic.com\/health\/hand-washing\/HQ00407\" >good old soap and water are your best defenses<\/a>. That old advice about not rubbing your eyes, nose or mouth was right on as well. A study published this fall linked <a title=\" Relative Contributions of Four Exposure Pathways to Influenza Infection Risk\" href=\"http:\/\/www.ingentaconnect.com\/content\/bpl\/risk\/2009\/00000029\/00000009\/art00011\" >nearly one third of flu risk to participants\u2019 hand to face contact<\/a>. If you\u2019re traveling and worry you might not have access to a washroom, I don\u2019t see a problem with taking along a bottle of natural sanitizer. At worst, it\u2019s simply an unnecessary measure. At best, it might give you some peace of mind. And, of course, if you\u2019re the one who\u2019s sick and you want to exercise some extra precaution\/courtesy, go ahead and use it as a backup to hand washing when you need to. All that said, I fear that hand sanitizers represent a growing trend to sterilize anything and everything when sometimes all we need is a little <a title=\"Going Grubby: Dirt, Dust and Dishevelment\" href=\"http:\/\/www.marksdailyapple.com\/going-grubby-the-primal-benefits-of-dirt-dust-dishevelment\/\" >dirt, dust and dishevelment<\/a>.<\/p>\n<p><strong>But let me turn it over to all of you.
