That was inaccurate &#8212; because, as Democrats were eager to say, it was never quite clear to them what President Obama actually wanted out of a health care bill.&nbsp; Today, the questions end. The President <a href=\"http:\/\/www.whitehouse.gov\/health-care-meeting\">has unveiled his own comprehensive health care bill<\/a>. It borrows heavily from the Senate&#8217;s bill, adds some White House ideas, throws in some Republican-sponsored amendments, and pronounces itself ready for inspection. <br \/>The topline highlights:<\/p>\n<p>&#8212; Proposes to cover 31 million Americans<br \/>\n who<br \/>\ndon&#8217;t have health insurance;<\/p>\n<p>&#8212; Creates a new federal facility to<br \/>\n help<br \/>\nstates crack down on insurance industry abuses and unfair rate<br \/>\nincreases;<\/p>\n<p>&#8212; Includes significantly ramped up efforts to crack<br \/>\ndown on<br \/>\nwaste and fraud within the Medicare\/Medicaid systems &#8212; this is a nod<br \/>\nto Republicans (Peter Roskam and Mark Kirk are behind proposals to do<br \/>\njust this);<\/p>\n<p>&#8212; Adds a Medicare tax of 2.9% on unearned income &#8212;<br \/>\nhitting the wealthy; it immediately closes the Medicare Part D doughnut hole gap &#8212;<br \/>\nsomething seniors should notice before the November 2010 elections if<br \/>\nthis gets through Congress;<\/p>\n<p>&#8212; Increases tax credits to families<br \/>\nto<br \/>\nhelp them buy insurance; it spends $11 billion on community health care<br \/>\ncenters;<\/p>\n<p>&#8212; Endorses but adds consumer protections to the Senate&#8217;s<br \/>\nproposal to grandfather in insurance plans that people want to keep; it<br \/>\nadopts the Senate proposal to require that Americans who don&#8217;t buy<br \/>\ninsurance pay a flat fee &#8212; but lowers the fee;<\/p>\n<p>&#8212; Provides $40<br \/>\nbillion to small businesses to help them defray the costs of providing<br \/>\nhealth benefits if taxpayers pay for the coverage; the president delays<br \/>\nby five years the Senate&#8217;s excise tax on so-called Cadillac insurance<br \/>\nplans;<\/p>\n<p>&#8212; Increases fees for brand name (as opposed to generic)<br \/>\ndrugs,<br \/>\ndepriving the pharmaceutical industry of an extra source of profits;<\/p>\n<p>&#8212;<br \/>\n Eliminates the Nebraska exemption for Medicare payments and adopts a<br \/>\nuniversal, phased-in approach to help states pay for the increased<br \/>\ncosts of expanding the Medicare program. 100% of the increased costs<br \/>\nwould be paid for three years; the bill turns the fee on medical<br \/>\nequipment providers in the Senate bill into an excise tax and delays<br \/>\nits implementation; it also delays by three years the tax on insurance<br \/>\ncompanies;&nbsp; <\/p>\n<p>&#8212; Finally, the bill gives the executive branch $1<br \/>\nbillion to<br \/>\nhelp efficiently implement the plan.<\/p>\n<p>The political highlights:<br \/>\nthere&#8217;s plenty in this bill for Democrats to like. But there&#8217;s no public<br \/>\n option. It&#8217;ll be easier to get unions on board because the excise tax<br \/>\nwill be delayed and watered down a bit; the bill delays implementation<br \/>\nof several provisions, which will annoy Democrats; it spends quite a bit<br \/>\n of time focusing on trying to rid Medicare of waste, fraud and abuse.<br \/>\nThere&#8217;s no nod to tort reform, so far as I can see. <\/p>\n<p>So &#8212; this<br \/>\nis just a bill. How does it become a reality?&nbsp; It relies on the idea<br \/>\nthat Democrats now really want to get something done. There is no public<br \/>\n option or a national exchange; the bill is still expensive and fairly<br \/>\ncomplicated; there are some carrots for those concerned about the<br \/>\nexpensive of the House bill. It&#8217;s hard to see how any Republicans will<br \/>\nsupport the bill, although the White House is framing it in a way so as<br \/>\nto force Republicans to defend their opposition to it during Thursday&#8217;s<br \/>\nBlair House health care summit.&nbsp; Since the first House vote, Democrats<br \/>\nare down at least three votes. <\/p>\n<p>The way forward: either<br \/>\nrank-and-file Democrats accept this bill &#8212; Obama&#8217;s bill &#8212; or they<br \/>\ndon&#8217;t.&nbsp; Republicans could suddenly discover an upside in supporting it,<br \/>\nwhich is doubtful, but if they could also reduce the number of<br \/>\nprocedural delays to help the Democrats speed its passage. If not, we<br \/>\nmight be faced with the spectacle of Republicans trying to drag the<br \/>\nvotes out through the midterms. This could benefit either party; it<br \/>\ndepends on whether Americans view this bill, which is basically a<br \/>\nproduct of the bills they don&#8217;t like, as something new.
