The collision caused a chain- reaction crash with two other cars.<\/p>\n<p>Montalvo, who lived in Rowland Heights, died at the scene. At least one other driver suffered trauma and had to be flown to a hospital by helicopter, said Lt. John Saleeby of the L.A.
