Mulligan, MD., Cleveland Clinic Foundation Metabolic Bone Clinic, <a href=\"http:\/\/\" style=\"font-family: Trebuchet MS;\">&#8220;<\/a><a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20200983\" style=\"font-family: Trebuchet MS;\">Taking vitamin D with the largest meal improves absorption and results in higher serum levels of 25-hydroxyvitamin D&#8221;<\/a>, <strong>J Bone Miner Res<\/strong> Feb. 8, 2010 (ahead of print)-<\/span><\/span><\/p>\n<p style=\"text-align: left;\">\n<\/blockquote>\n<p style=\"text-align: left;\">\n<p style=\"text-align: left;\">\n<p style=\"text-align: left;\"><a href=\"http:\/\/www.happyhealthylonglife.com\/happy_healthy_long_life\/2010\/03\/vitamin-d-at-dinner.html\">Click Here<\/a> for the web version of this post (and all the links) if you&#8217;ve received post this via email.<\/p>\n<p style=\"text-align: left;\">OK, we all know by now how important it is to raise our vitamin D levels to &#8220;optimally&#8221; around <strong>50 ng\/mL<\/strong>&#8211;or at the very least to 32 ng\/mL.\u00a0<em> If you need a refresher in the importance of vitamin D, click <a href=\"http:\/\/www.happyhealthylonglife.com\/happy_healthy_long_life\/2009\/10\/goodbye-sun.html\">here<\/a>, <a href=\"http:\/\/www.happyhealthylonglife.com\/happy_healthy_long_life\/2010\/02\/vitamin-d.html\">here<\/a>, and <a href=\"http:\/\/www.happyhealthylonglife.com\/happy_healthy_long_life\/2009\/12\/hollis.html\">here<\/a>.<\/em><\/p>\n<p style=\"text-align: left;\">But guess what?\u00a0 It&#8217;s easier said than done.\u00a0 I, for one, have found out how slowly those levels really do rise. So what&#8217;s a person to do&#8211;especially if you aren&#8217;t able to get out into the sun 3 times a week?<\/p>\n<p style=\"text-align: left;\">All I can say is, &#8220;Thank you Dr. Angelo Licata for devising this simple, straightforward research project that&#8217;s provided an answer for all of us!\u00a0 From now on, I&#8217;m taking my vitamin D with my dinner.\u00a0 No more taking it on an empty stomach, on the fly, in between meals!&#8221;<\/p>\n<p style=\"text-align: left;\">Licata and Mulligan figured that since vitamin D is fat-soluble, absorption just might improve if patients took their supplements with their largest meal of the day!\u00a0 How simple is that?\u00a0<a href=\"http:\/\/www.happyhealthylonglife.com\/happy_healthy_long_life\/2010\/03\/olive-oil.html\"> See the Iowa State salad dressing\/vitamin absorption study<\/a><\/p>\n<p style=\"text-align: left;\"><strong><span style=\"font-size: 14px; color: #bf5f00;\"><span style=\"font-size: 14px; color: #bf5f00;\">Who participated in this study?<\/span><\/span>\u00a0<\/strong> There were 17 patients, a mix of men &amp; women, with an average age of 64.5, and an average 25OHD level of 30.5.\u00a0 All were being seen at the clinic for vitamin D deficiency, and they were selected for this study because they <strong>were unable to achieve adequate increases in their vitamin D levels <\/strong>despite taking vitamin D supplements<strong>.<\/strong>\u00a0 Turns out, all of these patients were taking their vitamin D either on an empty stomach, or with a small meal, usually breakfast or lunch.<\/p>\n<p style=\"text-align: left;\">This was great mix of patients.\u00a0 Because of different factors, some were on &#8220;average&#8221; doses of vitamin D, and some were on megadoses.\u00a0 Some were taking D2.\u00a0 Some were taking D3.\u00a0 Some were taking an oil preparation.\u00a0 Some were taking a solid preparation.\u00a0 And all were having difficulty getting their levels up to optimal levels.<\/p>\n<ul>\n<li>One group was taking about 1,400 IUs of vitamin D3 (either oil or solid capsule) a day, with a baseline 25OHD averaging <strong>28.9<\/strong><\/li>\n<li>One group was taking 50,000 IUs of vitamin D2 (oil preparation) per week with a baseline 25OHD averaging <strong>31.3<\/strong><\/li>\n<li>One small group of 3 was taking &gt;50,000 (average dose of 183,333 IUs a week) IUs of vitamin D2 (an oil preparation) per week with a baseline 25OHD averaging <strong>32.4<\/strong><\/li>\n<\/ul>\n<p><strong><span style=\"font-size: 14px;\"><span style=\"color: #bf5f00; font-size: 14px;\">What were the participants asked to do?<\/span><\/span><\/strong>\u00a0 All 17 participants were told to keep taking their usual vitamin D supplement, but take it with their largest meal of the day, usually dinner.\u00a0 <\/p>\n<p><strong><span style=\"color: #bf5f00; font-size: 14px;\">And the envelope, please.<\/span><\/strong>\u00a0 After 2-3 months the participants had their 25OHD levels measured, and everyone ended up with an amazing average increase of around 50% over baseline in their vitamin D levels.\u00a0 <\/p>\n<ul>\n<li>The 1,400 IUs of D3 folks went from 28.9 before to <strong>45.4 ng\/mL<\/strong> after.<\/li>\n<li>The 50,000 IUs of D2 folks went from 31.3 before to <strong>48.3 ng\/mL <\/strong>after.<\/li>\n<li>The &gt;50,000 IUs of D2 folks went from 32.4 to <strong>48.7 ng\/mL <\/strong>after.<\/li>\n<\/ul>\n<p><strong>Big point:\u00a0 <\/strong>the type of vitamin D preparation taken made no difference&#8211;whether it was D2 or D3&#8211;oil or solid.\u00a0 All three groups achieved increases over baseline of around 50%.<\/p>\n<p><span style=\"color: #000000;\"><strong><span style=\"color: #bf5f00; font-size: 13px;\"><span style=\"font-size: 14px;\">Why concerns about taking high doses of vitamin D are unfounded.\u00a0 Why a <\/span>Doctors&#8217; Care is Critical.<br \/><\/span><\/strong><\/span><\/p>\n<p><span style=\"color: #737373;\"><span style=\"color: #000000;\">According to Drs. Licata and Mulligan<\/span><span style=\"color: #000000;\">, some patients clearly require very large doses of vitamin D to achieve a mid-range\u00a0 level.\u00a0 They recognize that many physicians are concerned about vitamin D toxicity at such high levels.<\/span><\/span><span style=\"color: #000000;\"><\/span><\/p>\n<blockquote>\n<p><span style=\"color: #737373;\">&#8220;However, it is not the dose prescribed alone which should be considered, but how much is actually absorbed.\u00a0 Some patients require very large doses to achieve mid-normal levels.\u00a0 <\/span><span style=\"color: #737373;\"><strong>Monitoring the serum level is critical.\u00a0 <\/strong><br \/><\/span><\/p>\n<p><span style=\"color: #737373;\">A recent study (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19307517\">click here<\/a>) indicates that serum concentrations of 25OHD <strong>only in the high-normal reference interval prevents non-vertebral fracture.\u00a0 (normal range: 31-80 ng\/ML)<br \/><\/strong><\/span><\/p>\n<p><strong><span style=\"color: #737373;\">Hence, striving to achieve this level (i.e. ~50 ng\/mL) may be optimal, rather than a level at the low-end of the normal range.<\/span><span style=\"color: #8b8b8b;\">&#8220;<\/span><\/strong><\/p>\n<\/blockquote>\n<p><strong>Thoughts about Citracal:\u00a0<\/strong> In light of Dr. Licata&#8217;s study, I couldn&#8217;t help but think about how many women depend upon getting their vitamin D from Citracal&#8211;the calcium\/vitamin D capsule that prides itself on being easy-to-take-and-absorbable on an empty stomach!<\/p>\n<p><strong>Limitations to the study:<\/strong>\u00a0 This study was small, there wasn&#8217;t a control group, and there wasn&#8217;t a way for the researchers to &#8220;know exactly how the patients took their supplements.&#8221;\u00a0 <\/p>\n<blockquote><p><span style=\"color: #8b8b8b;\">&#8220;But despite of these limitations, &#8220;the results are striking, and consistent across a heterogeneous group of patients (ie different disease states and different preparations and doses of vitamin D).\u00a0\u00a0<\/span><\/p>\n<p><span style=\"color: #8b8b8b;\">It therefore seems reasonable to ask patients to take vitamin D supplements with their largest meal, as it may be a cost-effective strategy that could very well help patients achieve optimal serum levels of 25OHD.&#8221;<\/span><\/p>\n<\/blockquote>\n<p><span style=\"color: #000000;\"><br \/><\/span><\/p>\n<\/p>\n<p style=\"text-align: center;\"><a href=\"http:\/\/www.happyhealthylonglife.com\/.a\/6a00e54fc8012e88330120a95f3059970b-pi\" style=\"display: inline;\"><img decoding=\"async\" alt=\"VitaminDlevels\" class=\"asset asset-image at-xid-6a00e54fc8012e88330120a95f3059970b \" src=\"http:\/\/www.happyhealthylonglife.com\/.a\/6a00e54fc8012e88330120a95f3059970b-350wi\" style=\"width: 350px;\"><\/img><\/a> <br \/> \u00a0 <\/p>\n<p> <\/p>\n<p style=\"text-align: left;\">\n<p style=\"text-align: left;\">\n<\/p>\n<\/div>\n<div class=\"feedflare\">\n<a href=\"http:\/\/feeds.feedburner.com\/~ff\/typepad\/happyhealthylonglife\/happy_healthy_long_life?a=a5Q3dLURqB0:7NWN-zHTPGM:yIl2AUoC8zA\"><img decoding=\"async\" src=\"http:\/\/feeds.feedburner.com\/~ff\/typepad\/happyhealthylonglife\/happy_healthy_long_life?d=yIl2AUoC8zA\" border=\"0\"><\/img><\/a>\n<\/div>\n<p><img loading=\"lazy\" decoding=\"async\" src=\"http:\/\/feeds.feedburner.com\/~r\/typepad\/happyhealthylonglife\/happy_healthy_long_life\/~4\/a5Q3dLURqB0\" height=\"1\" width=\"1\"\/><\/p>\n","protected":false},"excerpt":{"rendered":"<p>&#8220;In our practice, it is common to see patients treated with vitamin D supplements who do not achieve an appreciable rise in their serum 25OHD level after therapy despite large prescribed doses.
